NSW Population Health Survey

The Population and Public Health Division at the NSW Ministry of Health (NSW Health) has engaged Wallis Social Research to carry out the annual NSW Population Health Survey (PHS).
This survey includes questions about your health and lifestyle. Your answers will help NSW Health improve health services in your community.

When is the survey being conducted?
Wallis will begin inviting the NSW population to complete the survey from the 31 March 2025.
The survey will close on the 24 December 2025.
How long will the survey take?
The survey will take about 15-20 minutes to complete.
Completing the survey on behalf of a child only takes 10-15 minutes.
How do I take part?
Wallis might contact you by email, SMS (from mobile number 0480800556) or phone to participate in the survey or to complete the survey on behalf of your child. You will be asked to complete the survey one of two ways:
Online: You might receive a letter containing a QR code, you can scan this to complete the online survey
Over the phone: You might receive a phone call to complete the survey over the phone with one of Wallis' telephone interviewers
Who is carrying out this survey?
The NSW Population Health Survey is being carried out by Wallis Social Research, an independent social research company on behalf of NSW Health. To learn more about NSW Health and the NSW Population Health Survey visit www.health.nsw.gov.au/surveys

How was I chosen for the survey?
If you received a letter from us to complete the survey online, it is because your household has been chosen at random from a list of all household addresses across New South Wales. If we have called you to complete the survey over the phone, it is because we have sourced your phone number from a provider.
Can I be identified as a respondent?
No. Your answers are put together with the answers from other people and are not linked to you. Your individual answers to the questions will be kept confidential by Wallis Social Research, and by approved NSW Health staff and researchers.
Can I exit the survey and come back later?
Yes. You can exit the survey and come back to finish it later. Simply scan the QR code to log back into the survey or make an appointment to complete the survey over the phone at another time.
